How Tooth Gems Are Applied, Step by Step
A tooth gem is a tiny piece of jewellery — a crystal, a rhinestone, or a small gold charm — bonded to the front surface of a tooth. Done properly, the process borrows the exact same materials and technique a dentist uses to stick an orthodontic bracket to a tooth, which is why a well-applied gem is both secure and, importantly, reversible. Here is what actually happens from start to finish.
Step 1: Cleaning and isolating the tooth
Everything depends on a clean, dry surface. The practitioner first polishes the tooth to remove plaque and any film, then isolates it from saliva using cotton rolls or a cheek retractor. Bonding resin will not stick to a wet or contaminated tooth, so this unglamorous step is the one that most determines whether a gem lasts a week or a year.
Step 2: Etching the enamel
Next comes a gel containing a weak phosphoric acid, painted onto a spot smaller than the gem itself and left for a few seconds. This is the step that worries people, but it is the same mild etch used before every filling and every brace. It very slightly roughens the outermost layer of enamel at a microscopic level, creating tiny pits for the adhesive to grip. The gel is then rinsed off thoroughly and the tooth dried again, at which point the etched area looks faintly frosty.
Step 3: Priming and applying the bond
A thin bonding agent is brushed over the etched spot. This primer flows into the micro-pits and forms the chemical bridge between the tooth and the adhesive resin that will hold the gem. On a light-cured system the bond is not hardened yet — it stays workable until a curing light is used.
Step 4: Placing the gem
A small dab of composite resin — the same tooth-coloured material used for white fillings — is placed where the gem will sit. The practitioner picks up the gem with a fine applicator, sets it into the resin, and nudges it into the exact position and angle chosen beforehand. This is the moment placement is decided, so it pays to have agreed on the tooth and the spot in advance.
Step 5: Curing
A dental curing light is held over the gem for a number of seconds. The high-intensity light triggers the resin to polymerise and harden around the base of the gem, locking it in place almost instantly. With a self-cure resin there is no light; the material sets chemically over a couple of minutes instead. Either way, within a few minutes the gem is firmly bonded.
Step 6: Cleaning up the edges
Finally, any excess resin around the gem is trimmed and smoothed. A rough or overhanging edge is a trap for plaque, so a careful finish is part of doing the job well. The practitioner checks that floss passes cleanly beside the gem and that nothing catches the lip or tongue.
Why it does not damage the tooth
The key point is that nothing is drilled. No hole is made, and no healthy tooth structure is removed. The gem sits on top of the enamel, held by a resin that grips the etched surface. Can you do it at home?
Mail-order kits exist, and they are tempting because they are cheap. The problem is that they rarely include proper isolation or a real curing light, and some use craft glue rather than dental adhesive. A poorly bonded gem falls off quickly at best; at worst, the wrong glue irritates the gum or leaves residue that traps bacteria against the enamel.
